Insurance policy

We accept assignment of dental insurance benefits whenever possible. In order to process your insurance claim properly you will need to provide us with your dental insurance card, complete information on the primary insurance holder and proof of identification. The information provided will help us assist you with maximizing your dental insurance benefits. Your insurance plan may have a co-payment, the percentage of the fee paid by the patient. We will be glad to file pre-treatment estimates with your dental insurance carrier. All quoted co-payments are considered estimates only and are due in full on the date of treatment.  Regardless of insurance estimated coverage, our fees are charged to the patient not the dental insurance carrier. Many insurance carriers can be accessed online and allow you, as the policy holder to view your dental insurance information on coverage and benefits.

Please call our Insurance Clerk to discuss any questions you may have about your dental insurance coverage or our participation as a preferred provider in your dental plan.

Billing Policy

Our fees vary with the type of tooth and the complexity of treatment. We will be glad to give you an estimate of your charges prior to your treatment. Our fees are the same for all patients, whether or not they have dental insurance coverage. If you do not have dental insurance coverage our full fee is due at the time of service. For your convenience we accept VISA, MasterCard, Discover, and HSA cards as well as cash or checks.

Our office also offers a convenient extended payment plan through Care Credit patient payment plan. You may access this payment plan on the internet and pre-qualify yourself or call our office and we will assist you.
It is important for you to understand that our fees are charged to you and not your dental insurance carrier.  We will always be happy to file all insurance and accept assignment of benefits when possible. Patient remains responsible for their account regardless of insurance payment or estimated coverage.
